Birth certificate

The Idaho Bureau of Vital Records has no public counter, so order online through VitalChek (extra fee) or by mail. To order by mail, send a request form (English or Spanish), a copy of your photo ID, and a check or money order to Idaho Vital Records, PO Box 83720, Boise, ID 83720 0036. Missing items double the processing time.
