Birth certificate

The Indiana Department of Health says the fastest way is the local health department in the county where you were born, in person or by order. You can also mail State Form 49607 to the state (2 to 3 weeks), order online through VitalChek, or call 866 601 0891 any time. The first copy costs $10 and each extra copy in the same order is $4. Online and phone orders add fees.
