Quick answers

What do I need for a West Virginia ID card?

A first time applicant needs one proof of identity, one proof of Social Security number, and two proofs of West Virginia residency with your name and physical address. If you only lost your card and your address did not change, a duplicate needs one proof of identity and one proof of residency.

How do I get my birth certificate?

The Vital Registration Office at 350 Capitol Street in Charleston offers same day walk in service. A certified copy costs $12. You can also order by mail. Bring a photo ID, or two items from the list on the page. The office says exceptions can be granted case by case if you have none.
