How to get a Utah ID card, a birth certificate and your military papers when you are homeless: fee waivers from the Driver License Division and Vital Records, plus help from shelters.
Hi friend. Losing your ID is stressful, and it can block jobs, benefits and housing. Utah waives the fee for both a state ID and a birth certificate if you are homeless.
The Driver License Division waives the ID card fee if you give written proof that you are homeless. Get the Verification of Homeless Status form signed by a provider on the division's service provider list. Then go in person to any Driver License Office. You will need proof of identity and your Social Security number. The form is your proof of Utah address. Without a waiver the fee is $23.

The form is good for 60 days after it is signed. One ID card is issued per form, and a replacement needs a new application and a new form. Faxed or photocopied documents are not accepted. A temporary ID is issued the same day, and the final card is mailed in about 4 to 6 weeks. Booking an appointment online is preferred. Call 801 965 4437 or 888 353 4224.

Utah waives the fee for a certified copy of your own birth certificate if you are homeless, or if you are a youth in or aged out of state custody and under 26. You can order by mail, in person at a local vital records office, or online. You need the application, acceptable ID, and the Verification of homelessness form. By mail, the certificate is sent within 2 weeks if your papers meet the rules.

Only order from the official state site. The Office of Vital Records says third party vendors are not authorized to order vital records for you, and those requests will not be processed. A June 2026 notice on the site said orders were running about 6 weeks, so ask about the wait.
